Job Description:
Join a multidisciplinary care management team as a registered nurse coordinating services for selected patient populations. You will assess biophysical, psychosocial, environmental, discharge planning, and financial needs; monitor changes that may affect treatment; advocate for patients and families; collaborate with third-party payers; and coordinate discharge plans with the healthcare team and community resources. This travel assignment offers a focused role connecting clinical needs with practical support throughout the continuum of care.
