RN: Case Manager

Job Description:

Join us for an engaging 13-week contract as a Case Manager Registered Nurse in Salt Lake City, Utah. This role involves coordinating the entire case management process, from patient admission to post-discharge follow-up, ensuring smooth transitions and effective care delivery. You’ll work Monday through Friday, 8-hour days, from 8:00 AM to 4:30 PM, with a focus on facilitating patient progress, addressing family and patient needs, and collaborating with a multidisciplinary team.
