RN: Case Manager

Job Description:

This 13-week contract as a Lead Case Manager at North Florida Rehabilitation Hospital offers a unique opportunity to work in a state-of-the-art inpatient rehabilitation setting. You will oversee case management activities on a daily 8-hour shift, guiding patients through their recovery journey following severe injuries or illnesses. Your role will focus on coordinating and optimizing rehabilitation plans tailored to individuals with neurological conditions, orthopedic injuries, and other complex cases. This position combines leadership responsibilities with the rewarding challenge of helping patients regain their independence in a collaborative team environment.
