RN: Case Manager

Job Description:

Support patients and families as a travel healthcare case manager in Hermiston, Oregon. You will assess healthcare needs, develop and monitor individualized care plans, coordinate transitions between care settings, evaluate barriers to discharge, and connect patients with community resources. The role includes collaborating with physicians, therapists, social workers, insurers, caregivers, and post-hospital providers while advocating for patient preferences and access to appropriate services. You will also provide education, arrange referrals and authorizations, document outcomes, and participate in interdisciplinary care conferences. This assignment includes weekday and weekend coverage on a consistent daytime schedule.
