RN: Case Manager

Job Description:

This travel healthcare assignment centers on coordinating care from a patient’s entry into the healthcare system through post-discharge follow-up, including outpatient services. You will monitor progress toward desired outcomes, address patient and family needs, resolve barriers to effective care, and coordinate with payers, vendors, and internal care teams. You will also help establish, monitor, and carry out discharge plans. The schedule offers four 10-hour day shifts with rotating days off and weekend coverage required every weekend or every other weekend.

Shift: 10-hour days, 4 shifts per week, 07:00–17:30, with rotating days off
