RN: Case Manager

Job Description:

This travel assignment is for a seasoned registered nurse specializing in acute-care case management and discharge planning. You will coordinate care transitions, support discharge planning, and help patients move safely through the hospital stay and into the next stage of care. The schedule follows consistent weekday daytime hours, with weekend coverage scheduled according to department needs. Hospital case management experience is required; behavioral health experience alone does not meet the requirement.
