RN: Case Manager

Job Description:

Support adult patients throughout the hospital stay as a Case Manager Registered Nurse on a 13-week travel contract in Tucson. You will complete case management assessments, coordinate transitional plans of care from admission through discharge, arrange appropriate follow-up services, and help ensure safe transitions to home, home health, skilled nursing, or inpatient rehabilitation. This assignment is designed for an experienced acute inpatient RN case manager with adult emergency department and inpatient experience, EPIC proficiency, and prior travel nursing experience. Weekend coverage is scheduled periodically with a weekday off in return.

Job Specifics:

Location: Tucson, Arizona

Shift: Day shift, Monday through Friday, 8-hour days from 8:00 AM to 4:30 PM, with weekend coverage approximately every 4 weeks and a weekday off in compensation

Duration: 13 weeks at 40 hours per week
