RN: Case Manager

Job Description:

This travel RN case management assignment in San Antonio focuses on coordinating patient care across the continuum. You will complete comprehensive assessments, develop and implement care plans, support discharge planning, review resource utilization, and evaluate ongoing patient needs. Working Monday through Friday, you will collaborate with the Interdisciplinary Care Transitions team and other care partners to promote continuity, appropriate service delivery, and a smooth transition between care settings. This role offers a consistent weekday schedule and the opportunity to bring your expertise in acute-care case management to a welcoming Texas destination.
