RN: Case Manager

Job Description:

Join a travel assignment in Honolulu as a Case Manager RN supporting patients through coordinated care planning, utilization review, discharge planning, and transitions across the healthcare continuum. This role is designed for an experienced case management nurse who can bring at least two years of prior staff experience to a 16-week contract on Oahu. The schedule includes 40 hours per week with variable shifts, offering the opportunity to experience island living while contributing specialized nursing expertise to patient care coordination.
