RN: Case Manager

Job Description:

This 13-week travel contract is for a Registered Nurse Case Manager supporting an acute-care medical caseload in Kealakekua, Hawaii. You will coordinate care for medical patients, help guide each case through the healthcare system, and support appropriate transitions and follow-up. The assignment offers a weekday day-shift schedule with a 40-hour weekly commitment. A minimum of two years of experience is required.
