RN: Case Manager

Job Description:

Join us in a rewarding travel healthcare role focused on discharge planning and case management within a hospital setting in Hermiston, Oregon. This 13-week contract offers the opportunity to work weekdays from 8 am to 4:30 pm, including weekends, coordinating patient transitions, evaluating treatment needs, and advocating for patients and their families. Expect to develop and implement comprehensive care plans, facilitate communication across multidisciplinary teams, and connect patients to essential community resources.
