RN: Case Manager

Job Description:

This travel healthcare contract is an opportunity for a certified RN Care Manager to support patients across inpatient units in Lebanon, New Hampshire. You will manage a daily caseload, complete initial assessments, participate in interdisciplinary rounds, and collaborate with patients, families, caregivers, and the healthcare team to develop individualized plans of care. The role also includes coordinating post-discharge needs such as home health, inpatient rehabilitation, durable medical equipment, and nutritional services. You will communicate with medical teams and post-acute referral sources while documenting assessments and placing orders in the EPIC electronic medical record. This weekday assignment offers a consistent schedule with no weekends or on-call responsibilities.
