Schedule/Shift
Schedule includes 12-hour shifts from 7:00 AM to 7:00 PM and 9:00 AM to 9:00 PM, with every other weekend and 1-2 overnight on-call shifts per week.
Job Description
The LGH Hospital at Home program is launching on January 11th, seeking Registered Nurses to provide in-home patient care and assist with patient enrollment in the LGH hospital. Nurses will be primarily responsible for in-home visits and will work closely with the Hospital at Home team. Effective orientation in December will prepare the team for success in this innovative program.
Responsibilities
- Conduct patient assessments and provide nursing care during in-home visits.
- Assist with patient enrollment processes for the LGH hospital program.
- Maintain communication with hospital staff regarding patient care and progress.
- Participate in team meetings and training sessions as needed.
Required Experience
- Active Registered Nurse licensure.
- Experience in home health or community nursing preferred.
Certifications/Licensure
- Current CPR certification.
- State RN license required.
