Assignment Duration

This is a 6-month contract position.

Schedule / Shift

5 days per week, 8:00 AM to 4:00 PM, every other weekend with comp days during the week. Float required to different counties according to need.

Job Description

As a Registered Nurse in hospice care, you will perform patient assessments to determine appropriateness for hospice services, develop and ensure implementation of the plan of care, and oversee case management. You will also maintain communication with patients, families, and interdisciplinary teams while complying with hospice regulations and insurance guidelines.

Responsibilities

• Conduct patient assessments and develop tailored plans of care.
• Ensure proper documentation of all patient interactions and care plans.
• Manage and coordinate patient care services effectively.
• Supervise Home Health Aides as per standards.
• Participate in interdisciplinary team meetings and provide updates on patient progress.
• Adhere to infection control practices and ensure a safe environment for patients and staff.
• Participate in Performance Improvement activities and orientation for new staff.

Required Experience

Experience in hospice care and knowledge of hospice regulations and case management is preferred.

Certifications / Licensure

Current New Jersey RN license and Basic Life Support (BLS) certification are required.
