Job Description

The Case Management team at Wesley Medical Center is seeking a dedicated Registered Nurse to join their collaborative team. This role focuses on case management assessments, discharge planning, and supporting patients in step-down units, including CPCU and PCU. The CPCU unit has 14 beds dedicated to cardiac step-down patients, while the PCU unit caters to a diverse medical step-down patient population. The case management team benefits from consistent collaboration with physicians and mid-levels, fostering a supportive work environment.

Responsibilities

• Conduct case management assessments.
• Develop and implement discharge plans.
• Collaborate with healthcare professionals to facilitate patient care.
• Support a diverse patient population in an evolving healthcare environment.

Required Experience

• Minimum of 1-year current experience as a Case Manager RN.
• Active Kansas RN licensure.
• BLS certification.

Certifications/Licensure

• Active Registered Nurse license in Kansas.
• BLS certification required.
