Case Manager RN - Acute Care

Assignment Duration

This is a 13-week contract position.

Schedule

Shift: Day 4x10-Hour (07:00 - 17:30), with weekends required every other weekend.

Job Description

The Case Manager RN will be responsible for coordinating patient care within an acute care setting, ensuring effective discharge planning, and meeting the needs of diverse patient populations.

Responsibilities

• Manage case load and provide discharge planning for patients.
• Utilize Epic for charting and maintaining patient records.
• Coordinate care among interdisciplinary teams.
• Conduct continued stay reviews and pre-certification.
• Ensure compliance with CMS and The Joint Commission standards.

Required Experience / Certifications / Licensure

• Active AK RN License in hand required for auto-offer.
• BLS certification preferred.
• ACM certification preferred.
• Minimum of 1 year of acute care experience preferred.
• Experience with Epic charting system is required.
