Job Description

We are seeking a Home Health Physical Therapist to provide quality care to patients within a designated coverage area. The ideal candidate will manage an average of 4-6 visits per day, delivering evaluations and developing treatment plans in a home health setting.

Responsibilities

• Conduct evaluations, assessments, and develop individualized treatment plans.
• Measure patient progress and modify treatment plans accordingly.
• Collaborate with other healthcare professionals to provide comprehensive care.
• Document patient care accurately using Homecare Homebase software.

Required Experience / Certifications / Licensure

• Home Health and OASIS experience required.
• Homecare Homebase documentation experience required; candidates without this experience may be considered at a reduced rate with manager training.
• Valid Physical Therapist license.
