Job Description
The Licensed Practical Nurse (LPN) will assist with assessment, planning, implementation, and evaluation of patient care across all ages during their shift. The LPN is responsible for adhering to established standards of skilled nursing practice and will collaborate with the interdisciplinary care team while advocating for patients. Effective Person-Centered care and services are essential in this role.
The LPN primarily administers medications and treatments under the supervision of a Registered Nurse, takes vital signs, reports changes in patient conditions, coordinates care, and maintains clinical competence through ongoing education. The position involves documentation in the EMR/Point Click Care system. Familiarity with Point Click Care is preferred.
Responsibilities
- Assist with assessment, planning, implementation, and evaluation of patient care.
- Administer medications and treatments under the supervision of an RN.
- Monitor and document vital signs and changes in patient conditions.
- Provide Person-Centered care and services, advocating for patients.
- Coordinate care with the interdisciplinary team.
- Maintain clinical competence and participate in required education.
- Document patient care in EMR/Point Click Care accurately.
- Communicate effectively with nursing personnel regarding patient status.
Required Experience
Experience in skilled nursing care preferred.
Certifications/Licensure
- Valid LPN license in the state of New York.
- Completion of required health screenings and vaccinations, including Influenza and MMR.
- CPR certification is preferred.
