Job Description
The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents in a Long-Term Care facility, maintaining a collaborative relationship with nursing staff, physicians, and ancillary staff. The role requires knowledge of adult and geriatric care, emphasizing developmentally and culturally appropriate care plans that reflect residents' specific needs.
Responsibilities
- Collect and assess data relevant to residents’ physical and psychosocial conditions, ensuring findings are communicated to the Registered Nurse and interdisciplinary team.
- Contribute to creating individualized care plans that involve residents and families, ensuring they are developmentally and culturally appropriate.
- Evaluate resident outcomes and recommend necessary revisions to care plans, including referrals to address all aspects of resident care.
- Participate in ongoing education and mentoring of new staff, maintaining knowledge and competency in nursing practice.
- Work under the supervision of a Registered Nurse, providing leadership within the nursing practice setting.
Required Experience
1 year of experience as a Licensed Practical Nurse in a Long-Term Care facility.
Certifications/Licensure
- Current New York LPN License
- BLS certification (AHA or RQI)
