Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to assigned residents, working collaboratively under the supervision of a Registered Nurse (RN). This role requires the ability to assess and interpret patient data to meet the unique needs of adult and geriatric populations, while promoting a healthy and safe environment.

Responsibilities

• Collect and communicate ongoing data relevant to residents' physical and psychosocial conditions to the RN and interdisciplinary team.
• Participate in the development of age-appropriate and culturally relevant care plans; involve residents and their families in the planning process.
• Evaluate resident outcomes and recommend necessary revisions to the care plan, coordinating referrals as needed.
• Maintain knowledge and competency in nursing practice through continuing education and mentorship of new staff.
• Provide leadership in the delivery of care, ensuring safety and cost-effectiveness.

Required Experience

• Active New York LPN Licensure.
• BLS certification (AHA or RQI only).
• One year of experience in a long-term care facility as an LPN.

Certifications/Licensure

• Valid New York LPN Licensure.
• BLS certification required.
