Job Description
The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care for assigned residents in accordance with their specific needs and age-related requirements. The ideal candidate will work under the supervision of a Registered Nurse and will collaborate with the interdisciplinary team to ensure comprehensive care delivery.
Responsibilities
- Collect data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
- Develop, implement, and revise care plans in collaboration with the resident, family, and healthcare team.
- Maintain knowledge and competency in nursing practice through ongoing education and involvement in in-service programs.
- Demonstrate leadership in the professional nursing setting under the direction of the RN, while considering safety and cost-effectiveness in care delivery.
Required Experience
- 1 year of experience serving in an LPN capacity at a long-term care facility.
Certifications/Licensure
- Current New York LPN Licensure.
- BLS (AHA or RQI only).
