Full-time schedule: Monday to Friday, with 8-hour shifts from 7:00 AM to 3:00 PM.

Job Description

The Licensed Vocational Nurse (LVN) will provide care to patients in a home health setting, focusing on routine visits. Responsibilities include ensuring compliance with care plans, documenting patient progress, and coordinating with the healthcare team. The position may require travel within a coverage area of about a 60-mile radius around the facility.

Responsibilities

• Perform routine assessments and provide nursing care for patients in home health settings.
• Document patient care and progress in HomeCare HomeBase.
• Ensure that 32 points are met weekly for patient visits.
• Travel to different locations as needed within the service area.
• Communicate and collaborate with healthcare professionals to deliver high-quality care.

Required Experience

• Recent experience in home health is required.
• Experience with HomeCare HomeBase is necessary.

Certifications/Licensure

• Current LVN license is required.
