Job Description
This role involves providing home health care services in various areas of Houston, including Downtown, the Medical Center, West University Place, and the Galleria. The Licensed Practical Nurse will be responsible for routine patient visits and ensuring quality care while documenting via HomeCare HomeBase. LVNs are required to complete 32 points of visits per week.
Responsibilities
- Conduct routine home health visits to patients in designated areas.
- Document patient care and activities using HomeCare HomeBase.
- Maintain compliance with care protocols and standards.
- Collaborate with the healthcare team to provide effective patient care.
Required Experience
- Recent home health experience is required.
- Must achieve 32 points of visits per week.
Certifications/Licensure
Valid Licensed Practical Nurse (LPN) license is required.
