RN Case Manager

POSITION SUMMARY – RN Case Manager POSITION DUTIES – Provides comprehensive, patient-centered care using the nursing process to assess, plan, implement, and evaluate individualized treatment plans. The role involves coordinating with interdisciplinary teams, educating patients and families, advocating for patient needs and rights, documenting care, delegating appropriate responsibilities, and promoting safe, high-quality outcomes while addressing the physical, emotional, cultural, and educational needs of patients and families.

MINIMUM REQUIRED QUALIFICATIONS –

• CA RN license
• CPR
• 3+ years recent RN experience working in RN role
• 2+ year recent RN experience working in an specialty area
• Must have inpatient experience

LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8H/D (40hrs/wk) SYSTEMS – Epic
