RN Case Manager

POSITION SUMMARY – RN Case Manager POSITION DUTIES – Assesses patients in the emergency department, identifies individuals at high risk for readmission, coordinates discharge planning and community resources, facilitates care transitions, and collaborates with the interdisciplinary team to reduce avoidable hospital readmissions and improve patient outcomes.

MINIMUM REQUIRED QUALIFICATIONS –

• FL/compact RN license
• BLS
• 2 years recent case management experience in an inpatient acute hospital setting

PREFERRED QUALIFICATIONS – Care Management certification (ACM or CCM) LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 11a-7p
