RN Case Manager

POSITION SUMMARY – RN Case Manager POSITION DUTIES – Provides comprehensive, patient-centered nursing care in an outpatient clinic setting, supporting individuals and families across the continuum of care. This role utilizes the nursing process to assess, plan, implement, and evaluate care; administer medications and treatments; perform clinical procedures; and provide patient education and care coordination in collaboration with providers and interdisciplinary teams. Ensures accurate documentation, adherence to infection control and regulatory standards, and efficient clinic operations while promoting quality outcomes, patient safety, and culturally responsive care in a fast-paced ambulatory environment.

MINIMUM REQUIRED QUALIFICATIONS –

• High school diploma or equivalent
• Graduate of an accredited school of nursing
• AK RN license
• BLS
• Case Management experience in a clinic or hospital setting required within the last 2 years

PREFERRED QUALIFICATIONS –

• Bachelor’s degree
• Case Management certification

LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – Mon-Fri, 0800-1700 SYSTEMS – Meditech
