POSITION SUMMARY RN Case Manager

POSITION DUTIES Lead patient assessments, care planning, and discharge coordination with an interdisciplinary team to ensure quality outcomes and smooth transitions of care

MINIMUM REQUIRED QUALIFICATIONS - BSN, CA RN License, 5+ years clinical RN experience, 1+ year Case Management experience

PREFERRED QUALIFICATIONS - CCM or ACM certification, InterQual experience

LENGTH OF ASSIGNMENT 22 Weeks

SHIFT / HOURS PER WEEK Full-Time, 40 hours/week (8-hour shifts, 5 days/week)
