RN Case Manager

POSITION SUMMARY – RN Case Manager POSITION DUTIES – Collaborates with providers and interdisciplinary team members to coordinate care. Facilitates referrals, follow-up care, and continuity of services. Documents assessments, interventions, and outcomes accurately and timely in the electronic health record. Advocates for patient care and assists patients in navigating the healthcare system.

MINIMUM REQUIRED QUALIFICATIONS –

• Unrestricted AK RN license
• BLS
• Associate’s degree in Nursing from an accredited program
• 1 year of case management experience within the last 3 years

LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – M - F 3p-11p SYSTEMS – Meditech
