Details and requirements for this opportunity:

• Schedule: 9:00 am-6:00 pm
• Setting: Clinic
• Procedures/Cases: General Dentistry
• Credentialing Timeframe: 2-3 Weeks for New Provider. 1 Week if Already with Medicaid.
• Requirements: Active State License, Medicaid, Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Drug Enforcement Administration (DEA)
