We are seeking a licensed Pharmacist to join a patient-focused pharmacy team supporting individuals with complex medication needs. This role offers the opportunity to practice in a collaborative clinical environment while working closely with healthcare providers to optimize patient care.

What You'll Do

• Review and verify medication orders for accuracy, appropriateness, and safety
• Evaluate potential drug interactions, dosing concerns, and therapeutic opportunities
• Collaborate with physicians, nurses, and other healthcare professionals regarding medication management
• Maintain accurate patient medication profiles and documentation
• Provide clinical recommendations to support optimal patient outcomes
• Ensure compliance with state and federal pharmacy regulations
• Support medication preparation, dispensing, and quality assurance activities
• Participate in controlled substance and inventory management processes

What We're Looking For

• Active Pharmacist license in the state of practice
• Strong knowledge of pharmacotherapy and medication safety
• Excellent communication and clinical decision-making skills
• Ability to thrive in a fast-paced, team-oriented environment
• Experience in institutional, specialty, long-term care, hospice, or other clinical pharmacy settings is a plus
