This role offers flexibility, independence, and the opportunity to make a direct impact on patients in their home environment. The ideal candidate is comfortable working independently, conducting comprehensive assessments, and collaborating with interdisciplinary healthcare teams to ensure quality patient outcomes.Responsibilities:

• Conduct initial home care intake assessments for new patients.
• Perform routine and post-discharge follow-up visits in patients' homes.
• Complete comprehensive medical histories and physical examinations.
• Assess patient needs and determine eligibility for home care services.
• Develop and update individualized plans of care.
• Monitor patient progress and identify changes in health status.
• Collaborate with physicians, nurses, therapists, and care coordinators.
• Accurately document all patient encounters in the EMR.
• Provide patient and family education regarding disease management, medications, and treatment plans.
• Ensure compliance with all agency, state, and federal regulations.

Qualifications:

• Current New York State Nurse Practitioner (NP) or Physician Assistant (PA) license.
• Board Certification as an NP or NCCPA Certification as a PA.
